Appointment lagi!

Hari ni seawal 7.30 pagi bawak Adam gi DSH coz ada appointment with Dr. Razak (Paed Surgeon) at 8am abd Dr. Awal (ENT) at 11 am. Only me n Adam coz hubby cannot take leave. Reached DSH a bit late n Dr. Razak already at the OT coz dia ada kes, so need to wait until 10am after the operation. The reason bawak Adam kat sini, pertamanya nak check telinga whether dah 100% recovered dan keduanya to get 2nd opinion berkenaan Adam nyer hypospadias.

Telinga Adam dah 100% ok, tp Dr. Awal pesan just dalam tempoh 1 - 2 minggu elakkkan air dari masuk telinga. IOW takleh basah kepala, takleh swimming n yang sewaktu dengannya.

Hypospadias pulak, Dr. Razak dah bagi 2nd opinion as to the procedure, risks and the most important thing is the costs. He said in WCS and maximum to reserve is13k i.e. for operation, medicine n room. But if no complication the costs will be less than 10k. Compared to the costs quotedby Dr. Zul of SJMC is 6k but this one is only for operation not inclusive of medicine n room. So, next Moday have to call Dr. Zul again to check on the max costs. If lebih kurang dengan DSH, me n hubby will go for DSH coz Adam nyer record mmg dah ada kat DSH n dekat ngan umah. Pendek kata senang nak buat urusan.

What is HYPOSPADIAS?

Today is Adam's 1st appointment with Paed Surgeon Dr. Ahmad Zulkifli Laidin at Sime Darby Medical Centre (formerly known as SJMC) regarding his "Hypospadias". Dr. Zul was recommended by Adam's Paediatrician Dato' Azizi from DSH. After check-up, Dr. Zul said it just a minor defect and the urine opening is not too far away from its "original" place. So, the operation is not that difficult (hopefully so). Tentatively Dr. Zul fixed 14.01.09 morning as the operation date n Adam to be admitted on 13.01.09 evening. I already paid the deposit and booked the room. The costs??? 6k for the operation n plus ubat2 n maybe room charges lebih kurang 7k to 8k. BUT Dr. Zul said this type of operation normally not covered by any insurance co. coz it is considered as birth defect but SDMC will submit the report to insurance co., if lucky cover la, if not takde la. Whatever it is, klu tak cover pun, me n hubby will still buat kat SDMC coz the costs still cheaper than DSH.

A bit info about "Hypospadias" for my future reference...

hypospadiasWhat is hypospadias?

Hypospadias is a birth defect found in boys in which the urinary tract opening is not at the tip of the penis. Bending of the penis on erection may be associated and is known as chordee. Hypospadias occurs in about 1 in 100 to 1 in 200 boys. When we see a boy with hypospadias there is a 20% chance of finding this in another family member such as father or a brother. There are different degrees of hypospadias, some minor and others more severe. We name the types of hypospadias according to their anatomic location, but one must always determine whether or not there is associated chordee.

Why does hypospadias need to be repaired?

Surgical correction of hypospadias involves straightening of any chordee and then extension of the urinary tube (urethra) out to the tip of the penis (the glans). The ability to stand and urinate is important for boys. When the urethra opens before it reaches the glans a boy may be unable to stand and urinate with a direct stream. The youngster who has to sit down to urinate on a toilet is at a painful social disadvantage. A straight penis is necessary for satisfactory sexual function. Although this may not seem to be an important matter in childhood, this is a crucial concern later in life. Early childhood is generally the best time for correction of hypospadias and/or chordee.

How are hypospadias and chordee repaired?

It is recommended that the children have surgery to correct hypospadias at about six to nine months of age. Although we try and correct most children with one operation, about 10% will require a second surgical procedure to manage complications. If that is necessary, the second operation is delayed for one year. By performing the first operation early in life, both operations can be completed before the child is finished toilet training.

The operations performed to correct the hypospadias vary according to the degree of abnormality. The extra foreskin is sometimes needed to construct a new urethra. When the operation is completed, the extra skin is removed so the child will look circumcised. To protect the newly constructed urethra (neourethra) the urine is usually diverted with a stent (a silastic tube through the neourethra). We also leave a plastic dressing on the penis, which can be removed by the parent at home.
